Please call to make arrangements if Adult Hepatitis B is to be administered at
off-site clinic.

A $5.00 charge per
child, a parent must
accompany any child under 18 years of age - If a
relative 18 or older accompanies a child we must have written
authorization from the parent. At 2700 Johnson Street, Greenville,

Infant, pre-school immunization, and those required by school
systems are available. Please bring your
child's previous record if one is available, the social security
number for your child, and Medicaid number for the child if one is
available. Adult Immunizations:
- Hepatitis B - $32.00 Per
Injection (3
injections)
- Hepatitis A - $50.00 Per
Injection (2
injections)
- Hepatitis A/B - $50.00 Per Injection (3 injections)
- Meningococcal - $100.00 (injections)
- MMR - $ 55.00 Per Injection (2 Injections) (Born in 1957 or after)
- Td - $25.00 (Good for 10 years)
- Tdap - $ 40.00 (Good for 10 years)
- Tuberculin Test - $ 10
These prices may vary depending on the
availability of the vaccines.

Well Child Clinic - Nurse Clinic |
By
appointment only $5.00 charge and a parent must accompany the
child. Newborn screenings, nutrition and developmental counseling, and
immunizations if needed at that time. |

First Thursday of EACH month at Commerce Elementary,
while school is in session.
Immunization Clinic is from 1:30 p.m. to 3:00 p.m. - $5.00 per child and a
parent or relative must accompany any child under 18 years of age. If a
relative 18 or older accompanies a child we must have written
authorization from the parent. Please sign in 30 minutes prior to visit. |

Third Wednesday of EACH month at Alternative School on
Richmond Street, while school is in session. Immunization Clinic is from 1:30 p.m. - 3:00 p.m. -
$5.00 per child and a parent or relative must accompany any child under 18
