Proof of Ownership: Copy of the deed or contract for deed that shows ownership or purchase of the property along with Exhibit 'A'

Metes and bounds of the property
Accurate directions to the site from a major Highway or Farm to Market Road

Maintenance Contract, signed by landowner and installer.
“Aerobic Wastewater Treatment Systems Homeowners Information”
